On the medical form, if the answers to questions 1-10 are “No,” you do not need to fill out any other box. If you answer yes to any of the first 10 questions, please fill out the corresponding box indicated on the form. If you answer “Yes,” to questions 3, 5, or 10 OR to any of the questions in boxes A, B, C, D, E, F, or G, you will need a Physician Sign Off Medical Form to participate in any in-water activities. Please print the linked form to then be completed by a doctor and returned to us.
