Going on a trip
Every trip needs both of these.
Answered "Yes" on the medical form?
If all of questions 1–10 are "No," you don't need to fill out any other box. If you answer "Yes" to any of them, fill out the box the form points you to. If you answer "Yes" to questions 3, 5 or 10, or to any question in boxes A–G, you'll need a Physician Sign Off Form before any in-water activities. Print it, have your doctor complete it and return it to us.
Questions
Who can sign as my witness?
Anyone other than you and other than us. A fellow subway rider, someone stuck in traffic in the car next to you, the pizza delivery person, a co-worker.
Do I need a new medical form for every class?
No. If you've taken a class with us in the last 365 days, you don't need to fill out another medical form.
I'm taking two courses in the same week. How many forms do I need?
Every class, including specialties done the same week, needs its own General Liability Form.
